Job Summary:
The Client Coordinator works with clients, patients and active caregivers to identify open shifts, availability, and coordinate schedules. The Client Coordinator understands the needs, requirements, and preferences of all client types (patients, facilities, referral sources, caregivers, and staff) and optimizes staffing ratios. Through daily interaction with clients and caregivers, the Client Coordinator builds relationships with clientele to foster additional business opportunities.
Qualification and Education:
Essential Focus and Responsibilities:
1. Adhere and enforce the company vision, mission, and goals.
2. Recruit and retain private and state provided Clients.
3. Recruit and retain qualified Caregiver.
4. Conduct intake assessment, needs assessment, treatment planning, and reassessment services for new client.
5. Update and always maintain Clients records with 99 percent accuracy rate.
6. Build relationship with Authentic Heath Care Clients and active Caregivers to understand their scheduling needs, requirements, and preferences.
7. Coordinate and confirm schedules with clients (dr. appointment, shopping trip etc.) and Caregivers.
8. Document, monitor, and aid with client appointments.
9. Conduct quality assurance visits and monitoring activities for service delivery and documentation weekly.
10. Ensures all placements adhere to applicable compliance and contract requirements.
11. Partners with CEO, COO, Administrator, and Owner to debrief on staffing shortages, client satisfaction, and business development opportunities.
12. Assists with other duties as needed.

5. Salary: $75,000-100,000 annually.
6. Travel: 50 percent.
7. Remote work: 50 percent.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
The work environment and physical demands described here are representative of those required by an employee to perform the essential functions of this job with or without reasonable accommodations. Travel required fifty (50) percent of the time. Proof of the Full COVID-19 Vaccination (full series) is mandatory no later than thirty (30) days after accepting this position. Maximum use of a face mask is enforced daily.

Environmental Elements
Must work in an office environment with moderate noise levels, controlled temperature conditions, and no direct exposure to hazardous physical substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures. Ability to deal with difficult clients; dementia, etc.

The Employee will also be required to work in the field to maintain current and potential clients.
Conflict of Interest
During the term of the Employee’s active employment with the Employer, the Employee will not, directly or indirectly, engage or participate in any other business activities that the Employer, in its reasonable discretion, determines to be in conflict with the best interests of the Employer without the written consent of the Employer.
Contract Binding Authority
Notwithstanding any other term or condition expressed or implied in this Agreement to the contrary, the Employee will not have the authority to enter into any contracts or commitments for or on the behalf of the Employer without first obtaining the express written consent of the Employer.
Termination of Employment
Where there is just cause for termination, the Employer may terminate the Employee’s employment without notice, as permitted by law. The Employee and the Employer agree that reasonable and sufficient notice of termination of employment by the Employer is the greater of two (2) weeks or any minimum notice required by law. If the Employee wishes to terminate this employment with the Employer, the Employee will provide the Employer with the greater of two (2) weeks and the minimum required by law.
Non-Competition
The Employee agrees that during the Employee's term of active employment with the Employer and for a period of twelve (12) months after the end of that term, the Employee will not, directly or indirectly, as employee, owner, sole proprietor, partner, director, member, consultant, agent, founder, co-venturer or otherwise, solely or jointly with others engage in any business that is in competition with the business of the Employer within any geographic area in a fifty miles radio around Bridgeport CT, in which the Employer conducts its business.
General Provisions
By my signature, I acknowledge, that the Employer may make changes to the job title or duties of the Employee where the changes would be considered reasonable for a similar position in the industry or business of the Employer.

By my signature, I acknowledge that sharing and or reappropriation of the company ideas, Intellectual Property, and misuse of resources will lead to further disciplinary action up to and including termination and or a civil lawsuit.
